Upon admission to our facility, you (or someone you have designated to act on your behalf) will be required to complete various forms, which we must have in order to provide treatment and to bill Medicare or your insurance for treatment provided for your care.

This can take some time, and we require that you come at least one day prior to your schedule admission. Business office hours are Monday thru Friday, from 8 a.m. to 4 p.m.

PATIENT RESPONSIBILTIES

Each patient will need to bring 4 to 5 changes of clothes. Normal street clothes the patient wore prior to hospitalization are fine, although pullover tops and elastic waist pants may be easier for some patients initially. The patient will also need pajamas, toiletries, and rubber-soled (non-slip) shoes, such as tennis shoes. Also, any dentures, hearing aids or eyeglasses the patient wore prior to hospitalization will be needed. Our facility does not allow bar soap for hygiene purposes, so you may wish to purchase some liquid body wash for bathing.

The only other thing we ask for our patients to bring with them is a great attitude and a willingness to work hard in order to meet you and your family’s goals.
